Responsibilities
General Role:
Provide case management services to survivors/beneficiaries who require psychosocial support, ensuring safe, ethical, and effective care, either individually or in groups, while integrating with other community interventions.
Work Locations: Azaz – Safira – Manbij – Aleppo
Tasks and Responsibilities:
- Identify and assess cases requiring comprehensive psychosocial support.
- Conduct thorough psychological and social assessments of individuals.
- Classify cases according to the approved problem identification system.
- Develop and implement care plans in collaboration with beneficiaries, delivered individually or in group settings (support sessions, discussion groups, counseling).
- Refer cases to specialized services as needed (educational, vocational, legal, recreational, etc.).
- Develop and regularly update a community service map to ensure effective referrals.
- Monitor and review care plan implementation and adjust based on outcomes.
- Document all case management activities according to organizational policies.
- Maintain confidentiality and privacy of beneficiary files.
- Prepare required periodic reports and submit them to relevant authorities.
- Contribute to psychosocial awareness sessions and focused group discussions as needed.
